Why camps and youth programs need a signed waiver
When a child participates in a camp or sports program, the organization is responsible for supervision and safety during the activity. A signed waiver from the parent or guardian confirms that the parent understands the activities their child will participate in, acknowledges the physical risks involved, and authorizes the program to seek emergency medical care if something happens.
For camps that run week-long or multi-week sessions, the waiver is also a practical document. It collects emergency contacts, health information, and the guardian's contact details in one place, which staff need quick access to during the program.
What this template includes

1. Description of Camp and Sports Activities
I understand that participation may include camp programs, sports clinics, drills, games, conditioning, transportation between activities if offered, and related recreational or instructional activities.
2. Participant or Parent/Guardian Permission to Participate
I represent that I am either the participant or the legally authorized parent or guardian of the participant. I give permission for participation in the listed activities subject to the terms of this document.
3. Assumption of Risk
I understand that camp and youth sports activities involve inherent risks, including falls, collisions, contact with equipment or other participants, weather-related risks, and other injuries that may be serious. I voluntarily accept and assume those risks to the extent permitted by law.
4. Health Information and Emergency Medical Authorization
I represent that I am responsible for providing accurate health information, allergies, medications, and emergency contacts. If emergency care is reasonably necessary, I authorize staff to contact emergency services and obtain care for the participant.
5. Rules, Equipment, and Participant Conduct
I agree that the participant will follow program rules, use equipment only as instructed, and comply with directions from staff, coaches, and supervisors.
6. Release and Waiver of Liability
In consideration of participation, I release and hold harmless the program, its operators, staff, volunteers, contractors, and affiliates from claims arising out of participation, except to the extent prohibited by law.
7. Indemnification and Responsibility for Loss or Damage
To the extent permitted by law, I agree to be responsible for loss or damage arising from intentional misconduct, reckless behavior, or failure to follow stated rules.
8. Photo and Video Consent (Optional)
I understand that the program may request permission to use photographs or video from camp sessions, games, or events for operational or promotional purposes.
